Before Placement | After Placement |
□ Please ensure that your contact information is up-to-date on our ZSFG Preceptor Contact List, schools, and students will be referencing this list for placement opportunities. Contact DET to update information. □ If a placement opportunity is available and a student/school contacts you regarding this opportunity, please ensure that the student’s school and program are on the Approved List of Schools and Programs. □ If the student’s school and program are not on the Approved List or if you wish to have a school or program added to our Approved List, please request the school coordinator to fill out the Student Placement Agreement Form. □ Once a school or program is on the approved school lists, it is the preceptor’s responsibility to coordinate the student placement. |
□ Preceptors must attain a completed copy of the Student Placement Form from the Student □ Preceptors must attain a consented copy of the COVID-19 Additional Student Orientation from the Student. □ Preceptors must attain a copy of the Privacy and Compliance certificate from the Student. Click here for Privacy and compliance training instructions. □ Preceptors are solely responsible for the documentation & completion of the student placement process. □ Preceptors must retain a hard or electronic copy of any required forms for three years. □ It is the responsibility of a preceptor to provide instruction, supervision, and or mentorship to fulfill the student’s educational experience. Orientation Requirement □ Preceptors must ensure that all students complete the mandatory Online Orientation. Preceptors must ensure that all Clinical Students complete an online orientation before starting their clinical rotation or placement. Preceptors must ensure that all Non-Clinical Students complete an orientation within 30 days of their start date. □ The preceptor must attain a copy of the Certificate of Completion of the Online Orientation from the student. Identification Badge □ Preceptors must ensure that students are wearing their official school photo identification badges above the waist and at all times. □ If a student doesn't have an official school photo identification badge, they will require a ZSFG-issued identification badge. Please follow these steps: Badging Process: Step One: Provide the student a letter that includes the student/faculty full name, department/unit/clinic, student title (required), and start/end date (required) on official letterhead with the preceptor or supervisor’s signature. Step Two: The preceptor contacts Human Resources at 415- 206-5538 to schedule an appointment or sends the student to during drop-in Hours (M/W/F 10:00-10:30 am and T/Th. 2:30-3:00 pm). Step Three: Assure that the student brings valid photo identification to the appointment. HR is located at 2789-25th St. 3rd Floor, Rm 339 |
